Applicants should complete the online application, proposal describing their project and indicate which objective(s) they are addressing, and provide scanned files of the requested attachments, and submit all items online no later than August 31.
Receipt of all applications will be acknowledged via an automated e-mail message following your application submission online. During the review of your application, Foundation staff may contact you for additional information.

Approval from CDA Foundation staff is required prior to accepting faxed, mailed, or e-mailed proposals. If you are experiencing difficulties submitting the online application, please contact the CDA Foundation staff via e-mail at jolene.murray@cda.org to request an alternative submission method and a Word document of the grant application will be sent to you for completion.
