Nominate
Submit your online nomination packet and all requested and applicable attachments (scan into appropriate file format, PDF preferred, to attach to nomination packet) online no later than October 31.
Receipt of all nominations will be acknowledged via an automated e-mail message following your nomination packet submission online. During the review of your nomination, Foundation staff may contact you for additional information.
Please keep individual attachments under 300k. Please click here for scanning tips.
Approval from CDA Foundation staff is required prior to accepting faxed, mailed, or e-mailed nominations. If you are experiencing difficulties submitting the online application, please contact the CDA Foundation staff via e-mail at jolene.murray@cda.org to request an alternative submission method and a Word document of the nomination application will be sent to you for completion.
