Application Guidelines

The following guidelines are designed to facilitate the administration of the CDA Foundation Bettie Underwood Dental Assisting Scholarship.

Eligibility for Scholarships
Any individual who is interested in becoming a dental assistant and is currently enrolled in a state-approved program is eligible to apply for a $1,000 scholarship. Must be able to provide a letter of enrollment from school, transcript of grades, and proof/certification of hours of volunteer community service/involvement, no exceptions. Applicants must demonstrate financial need, responsibility/leadership, volunteer community/organization involvement and any outstanding academic achievements. Please note that internships or any other non-paid services that are part of a program curriculum or requirement do not qualify as volunteer community service/leadership under this scholarship program. Applicants should possess a strong desire for a career in the dental field.

Policy on Awarding Scholarships
The CDA Foundation will manage the program and award a $1,000 scholarship to the student recipient. The scholarship can be paid to the recipient or to the school (at the student’s request).

Handling of Applications
Complete (select the type of scholarship you are applying for at the top of the online application form) and submit your online application and all requested and applicable attachments (scan into file format to attach to application packet) no later than March 31. Upon receipt, applications will be reviewed by the Scholarship Selection Committee and approved by the CDA Foundation Board of Directors. Once selections are approved, recipients must sign a student agreement, and check(s) will be processed. All information received in connection with a scholarship application will be regarded as confidential by the CDA Foundation.
