Budget

(#1 below is a requirement for all program types, and # 2-3 is a requirement for new programs only.)

  1. Line item budget for total program cost that clearly identifies program related costs, personnel/salary, facilities, curriculum development, indirect costs, and revenue sources (please use only the budget template provided within the online application form to capture and submit this information);
  2. Method by which a dental society will be pledging financial support, whichever is less (please use the check box option in Section VIII of the online application to identify this)
    1. $300 per dental society member, or
    2. $300,000. (Requirement for new programs only.)
  3. Society’s plan to garner resources for this program. (Requirement for new programs only.)

Project Collaborators and Funding Partners

(Requirement for all program types.)

  1. Provide the agency/organization name, contact person, title, address, phone, e-mail address, and the role that they will play. Additionally, for new programs identify if they are a funding partner and the role they will play in the development of the program.

Written Letter of Commitment

(Requirement for new and innovative program types only. However, if an existing program is expanding its current space a letter confirming the expansion is required.)

  1. Letter from a private or public educational institution or business indicating its intention to serve as the dental hygiene umbrella organization; and
  2. Letter that a physical site for the program has been determined and that a preliminary agreement has been reached regarding its use as a dental hygiene educational facility.

Most Recent Tax Return or Audited Financial Statement

(Requirement for all program types.)

IRS Determination Letter 501(c)(3)

(Requirement for all program types. However, if applying for a new program and the proposal is from an unincorporated collaboration, tax exemption verification and a letter of agreement signed by an eligible fiscal sponsor are required.)

Review and Selection Process

Proposal Selection Criteria – New applicants and applicants who have not received a dental hygiene grant in the last five years will receive priority consideration.

Proposals that can clearly and concisely demonstrate the following characteristics will have a greater opportunity for funding.

  • Increases the number of hygienists
  • Demonstrates workforce need for allied personnel in the geographical area
  • Community-based and/or community-driven programs
  • Demonstrates established partnerships and collaborations
  • Encourages creative solutions to identified challenges
  • Includes direct participation by local dental society(s)

Funding Restrictions

  • Grants are made to organizations that are exempt from federal taxes under Section 501(c)(3) of the IRS Revenue Code or government agencies. If a proposal is from an unincorporated collaboration, tax exemption verification and a letter of agreement signed by an eligible fiscal sponsor are required.
  • Indirect costs may not exceed 15% of project direct costs.
  • Grants are not made for supporting conferences and special events (i.e. fundraising dinners)
  • Grants are not made for supporting lobbying and/or political campaigns or endorsements
  • Grants are not made for supporting organization budget shortfalls
  • Grants are not made to individual
