Application Guidelines

CDA Foundation recognizes that the need for dental supplies and materials is crucial to providing access to care for all Californians, particularly in underserved and culturally-diverse communities. The CDA Foundation in partnership with Henry Schein, Inc., addresses this issue through the dental materials and supplies grants, which awarded recipient organization (nonprofit or clinics in underserved communities) receive donated supplies and materials that are necessary for their establishment to operate and provide care.

Selected organizations will receive between $5,000 - $25,000 of goods per year for two years. The only cost to a recipient organization is the shipping cost of the goods.

Application Eligibility and Process

Support is provided to nonprofit organizations and clinics in both the public and private sectors. Federally qualified health centers, organizations in health professional shortage areas and dental professional shortage areas are of particular interest.

Before applying, you may want to be prepared to attach the following documents (in PDF, MS Word or Excel formats ONLY) in order to complete your application:
a. Organizational Operating Budget
b. IRS Determination Letter
c. Most Recent Tax Return Form 990 or Equivalent (if government entity, attach brief letter indicating no tax return required)
d. Copy of Organizational Letterhead with Board Members (if government entity, attach brief letter indicating no board established)
e. Recommendation Letter From Health Professional (1 page maximum)
f. Annual Report (if government entity, attach brief letter indicating no annual report prepared/created)

Review and Selection Process

Applications are reviewed once per year. The open application submission period is January 1 - March 31 annually. During the review of your application, CDA Foundation staff may contact you for additional information. Recommended organizations will be forwarded to the Henry Schein Donation Committee for final selection. Successful recipients will be contacted directly by Henry Schein to execute a donation agreement and scheduling of deliveries.
The timeline for the grant cycle process is:

  • January 1 - March 31 – open application submission period
  • October 30 – grant process results announced via letters to all applicants
  • March 31 the following year - New recipients can expect first materials delivery

Incomplete applications will not be reviewed.

Application Selection Criteria

Applications that can clearly and concisely demonstrate the following characteristics will have a greater opportunity for funding:

  • Relevance to the CDA Foundation’s mission
  • Community-based and/or community-driven organization
  • Demonstrates established partnerships and collaborations
  • Ability to use and dispense a high volume of material and supplies
  • Provides programs to underserved and diverse clients
