Applicants should complete the
online application, proposal describing their mobile dental project and
indicate which objective(s) they are addressing, and provide scanned files of
the requested attachments, and submit all items online no later than the
deadline date for the funding cycle you are applying for. The three semi-annual
application deadline dates for these grant review cycles are March 31, July 31, and October 31,
and recipients for each funding cycle will be announced in June, September and
December month-endings.
Receipt of all applications will be acknowledged via an automated e-mail
message following your application submission online. During the review of your
application, Foundation staff may contact you for additional information.

Approval from CDA Foundation staff is required prior to accepting e-mailed proposals. If you are experiencing difficulties submitting the online application, please contact the CDA Foundation staff via e-mail at jolene.murray@cda.org to request an alternative submission method and a Word document of the grant application will be sent to you for completion.
